Patient follow-up and education are the keys to successful disease
management, and improved overall quality of life. The GOLD1
Initiative identified patient education as one of the key factors
lacking in the diagnostic group of COPD. The AARC2
has, for many years, advocated education and ongoing evaluation to
help patients with lung disease achieve their maximal quality of
life. This is the standard we at HomeTown Oxygen use to develop and
deliver the very best care we can to the patient. Respiratory
disease is the “core” of our business. In order to meet the
needs of this population we have developed and implemented the
C.O.R.E.
family of programs.
C.O.R.E. Basic Program: No Order Required. An entry
level Patient Education and Evaluation provided at the set-up of
all respiratory equipment. Additionally, regular phone follow-up
is used to identify any needs or problems.
C.O.R.E RT Assessment Program: Order Required.
Includes all of CORE Basic plus a comprehensive
clinical respiratory assessment.
C.O.R.E. COPD Program: Order Required. HomeTown
Oxygen’s COPD disease management program which incorporates the
education modules found in most pulmonary rehabilitation
programs. The program provides outcome measurements to the
physician.
C.O.R.E. Sleep Program: Order Required. A
CPAP/Bi-Level program that places emphasis on patient compliance
through education and consistent follow-up.
C.O.R.E. Cardio Program: Order Required. Designed to
help identify and treat patients with apparent cardiac or
vascular conditions caused by an underlying respiratory
condition. Through the use of Webox® nocturnal oximetry,
conveniently delivered to and picked-up from the patients home,
we help to provide diagnostic values to identify needs for
treatment.
